Job Summary:

We are seeking an experienced Advanced Practice Infectious Disease Provider to join our team at AbsoluteCare Inc. As a key member of our care team, you will provide high-quality patient care, manage chronic infectious diseases, and work collaboratively with our multidisciplinary team to deliver comprehensive care.

Key Responsibilities:

  • Provide patient consultation, primary management of people living with HIV and other chronic infectious diseases
  • Develop diagnostic and treatment care plans, educate patients about their illnesses, and advise on specialist services
  • Work in a team environment, process feedback, and facilitate open discussion to deliver whole-person care
  • Implement plans of care in collaboration with patients, their families, and other team members
  • Develop strong transitional care plans and track patients along the continuum of care

Requirements:

  • Unrestricted Advance Practice License in the state of practice
  • Credentialed by AAHIVM
  • Minimum of 2 years' experience practicing as an Advance Practice Provider
  • Experience in population health management and working with psychosocially and medically complex patients
