Job Summary:

ECHN seeks a skilled RN Clinical Manager to lead our patient care team in a dynamic home health agency setting. As a key member of our leadership team, you will be responsible for managing professional and paraprofessional nursing services, ensuring high-quality patient care, and promoting a culture of excellence within our organization.

Key Responsibilities:

  • Assess and evaluate community needs, agency needs, and agency resources to inform staffing decisions and ensure effective patient care.
  • Make recommendations to the Hospice Director on staffing needs and requirements, ensuring a well-qualified team is in place to meet patient needs.
  • Ensure clinical competency of staff assigned, providing ongoing education and training to maintain high-quality patient care.
  • Responsible for accurate and timely assessment and evaluation of performance of staff assigned, identifying areas for improvement and implementing corrective actions as needed.
  • Participate in weekly Interdisciplinary Team Meetings, staying up-to-date on new trends and issues in patient care and sharing knowledge with the team.
  • Provide ongoing case evaluation to ensure clinical outcomes, documentation, and billing requirements are met, identifying opportunities for improvement and implementing changes as needed.
  • Collaborate with Hospice RNs and other disciplines to discuss patient care, progress on goals, coordination of services, and plans for ongoing service or discharge.

Requirements:

  • Registered Nurse with an active license to practice nursing in Connecticut.
  • Master's degree from a program approved by the National League for Nursing or the American Public Health Association, or a baccalaureate degree in nursing with a minimum of 3 years of full-time clinical experience in nursing, at least one of which was in a home health agency.
  • Evidence of certification by the American Nurses' Association as a community health nurse or completion of at least six credits received within two years in community health nursing or six credits in health care management from an accredited college or university program or school of nursing.

Preferred Qualifications:

  • Hospice/Palliative Certification.
