Job Title: Patient Engagement Coordinator

We are seeking a highly motivated and compassionate Patient Engagement Coordinator to join our team at Granite Recovery Centers. As a Patient Engagement Coordinator, you will play a vital role in supporting patients throughout their treatment journey.

Job Summary:

The Patient Engagement Coordinator is responsible for engaging patients on a daily basis by providing activity services, crisis intervention, and connecting with the various interests of the patient as a means of engagement. This role requires a strong understanding of the patient's needs and the ability to provide individualized support to ensure their success in treatment.

Key Responsibilities:
  • Provide daily support to patients through activity services, crisis intervention, and engagement activities
  • Develop and implement individualized engagement plans to meet the unique needs of each patient
  • Collaborate with the clinical team to ensure patients receive comprehensive care and support
  • Document patient interactions and progress in the electronic medical record (EMR)
  • Assist with the planning and execution of engagement activities throughout the month
  • Provide crisis intervention and support to patients as needed
Requirements:
  • High school diploma or equivalent required
  • Previous experience working with substance abuse and co-occurring disorders preferred
  • Ability to work in a fast-paced environment and adapt to changing circumstances
  • Strong communication and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information
