Join Our Team as a Clinical Assessment Advisor

At CenterWell Home Health, we are dedicated to providing personalized, comprehensive home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization. As a Clinical Assessment Advisor, you will play a vital role in our care teams, working closely with patients, families, and healthcare professionals to ensure optimal clinical outcomes.

Key Responsibilities:
  • Conduct initial and ongoing evaluations of patient needs, identifying areas for improvement and developing personalized care plans.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
  • Coordinate available resources to manage care plans, ensuring stated outcomes are achieved.
  • Periodically reassess patient needs and revise care plans as necessary.
  • Promote and coordinate communication between team members, attending physicians, administrative staff, referral sources, and external case managers to ensure appropriateness of care.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of one year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Excellent interpersonal skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Knowledge of clinical policies and procedures and ability to implement.
  • Knowledge of clinical structure of PDGM.
  • Must read, write, and speak fluent English.
  • Must have good and regular attendance.
  • Approximate percent of time required to travel: 60%.
