As a Social Worker, you will play a vital role in assessing patients' psychosocial distress and helping them navigate the complexities of home health and hospice care.
Key Responsibilities:
- Assess patients' psychosocial distress and develop plans to address their emotional, financial, and social needs
- Collaborate with treatment teams to coordinate community resources and services for patients and their families
- Provide education and support to patients and their loved ones on managing the challenges of home health and hospice care
- Develop and implement individualized plans to address patients' unique needs and goals
Requirements:
- Master's degree in Social Work from an accredited institution
- Current licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LSCSW) in the state of Kansas
- Knowledge of principles, methods, and techniques in performing general social work services
- Ability to adapt and modify standards of practice for care of specific medical cases
