Job Title: Hospice Registered Nurse Case Manager

About the Role:

This is a full-time position that requires a strong commitment to delivering exceptional patient care. As a Hospice Registered Nurse Case Manager, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare teams to ensure seamless care delivery.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to identify care needs and develop personalized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with healthcare teams, including physicians, social workers, and other healthcare professionals, to ensure comprehensive care delivery.
  • Coordinate with community resources and services to support patients and families in their homes and communities.
  • Document patient care and progress in accordance with regulatory requirements and organizational policies.
Requirements:
  • Current licensure as a Registered Nurse in the state of Oregon.
  • Graduate of an accredited nursing program.
  • Minimum of two years of experience in hospice or home care nursing, with a focus on community health education and experience.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to delivering exceptional patient care.
