We are seeking a highly skilled LPN: Licensed Practical Nurse to join our team at St. Margaret's Center, a pediatric and young adult skilled nursing facility. As a LPN: Licensed Practical Nurse, you will provide direct care and treatment to individuals in assigned programs through a variety of nursing procedures and medication administration.
Key Responsibilities- Provide direct care and treatment to individuals in assigned programs through a variety of nursing procedures and medication administration.
- Respond and assist in medical emergencies and take appropriate action with guidance from RN.
- Observe and monitor individuals with specific health problems and those at risk and report pertinent findings to RN.
- Document all current health-related information in nurses' notes and/or medical record.
- Check nursing equipment as indicated.
- Assist RN in annual assessment of consumers in day programs.
- Keep medical files current and complete.
- Attend nursing/program meetings as scheduled.
- Communicate with other service providers, program staff, administration, etc., re: changing medical issues.
- Current and valid NYS LPN License.
- Minimum of one year acute or long-term care experience, preferred.
- Must be 18 years old.
- Must be able to lift a minimum of 50 pounds.
