About the Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Albany, New York. As a member of our healthcare network, you will play a vital role in providing direct care and treatment to individuals in assigned programs.

Responsibilities
  • Respond to medical emergencies and take appropriate action
  • Observe and monitor individuals with specific health problems and report pertinent findings to RN
  • Document all current health-related information in nurses' notes and/or medical records
  • Assist RN in annual assessment of consumers in day programs
  • Keep medical files current and complete
  • Attend nursing/program meetings as scheduled
  • Communicate with other service providers, program staff, administration, etc.
Qualifications
  • 1+ year of experience in an Acute Care, Long Term Care, Outpatient and/or Skilled Nursing setting
  • Bachelor's Degree
  • NYS Licensed Practical Nurse (LPN)
  • Comfortable handling Adolescent and Pediatric population
  • Solid assessment, clinical, and documentation skills
  • Patient-oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized
