Job Summary

Cephas Consultancy Services Private Limited is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in a Long Term Care setting. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Administer medications and treatments as ordered, and accurately record resident responses
  • Transcribe physician orders and complete monthly summaries as scheduled
  • Review care plans with nursing assistants and update them to reflect resident changes
  • Notify the RN Supervisor of significant changes in resident condition and report changes to the resident's chart
  • Communicate resident transfers to family members and department heads, and complete transfer forms
  • Update care plans with clear, concise, and time-limited goals to reflect resident condition and direct the team to implement approaches
  • Assist physicians with rounds, examinations, and treatments as needed
  • Collect laboratory specimens and arrange for delivery to the hospital with accurate and complete requisitions
  • Document laboratory work in PCC and notify the RN Supervisor of resident death
  • Encourage resident participation in scheduled activities and report changes in resident condition, care plan changes, and special assignments to the team
  • Assist the RN Supervisor with annual evaluations of nursing assistants and write commendations and counseling records as needed
  • Accurately complete incident reports and assist team members in writing reports
  • Document weights and vital signs and follow up as needed
  • Clean and stock treatment trays daily and prepare the daily report for the wing
  • Count narcotics with another nurse at the beginning and end of the shift and maintain an accurate inventory of resident belongings
  • Maintain a clean, safe, and healthy environment and report potential safety hazards
  • Supervise and direct resident care by nursing assistants to ensure the prescribed plan of care is carried out
  • Perform additional duties as directed by the supervisor
Requirements
  • Good physical health and mental alertness to perform job functions and complete work assignments in a timely manner
  • Ability to pass employment physicals and as required
  • Knowledge of principles of good body mechanics
  • Have physician permission to perform duties outlined in the transitional duty job description
  • Must be free of communicable disease
