About the Position

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment provider. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks.

This provides unparalleled insight into patients' overall health and well-being, which can be challenging to capture during routine office visits. By working with Matrix, you will have the opportunity to make a meaningful impact on patients' lives and contribute to improving the quality of care.

Responsibilities
  • Conduct comprehensive health assessments in patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks.
  • Develop and implement personalized care plans to address patients' unique needs and health goals.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on healthy lifestyle choices, disease management, and medication adherence.
  • Document all assessments, care plans, and patient interactions in accordance with Matrix Medical Network's policies and procedures.
Requirements
  • Masters Degree in Nursing or Physician Assistant Studies required.
  • Current RN and NP/PA licensure in the state of practice, including prescription authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
  • Ability to travel to patients' homes and other community environments.
