About the Position

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment provider. As a member of our team, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.

During a visit that can last up to one hour, you will review and observe patients' current health, medical history, medication adherence, social environment, and other risks. This provides a unique opportunity to identify potential health issues and develop personalized care plans to improve quality of life.

Responsibilities
  • Conduct comprehensive health assessments in patients' homes, reviewing and observing their current health, medical history, medication adherence, social environment, and other risks.
  • Develop personalized care plans to improve quality of life and address potential health issues.
  • Collaborate with primary care physicians, case managers, social workers, and other healthcare professionals to ensure seamless care coordination.
  • Provide education and support to patients and their families, empowering them to take an active role in their healthcare.
  • Document patient information and care plans accurately and efficiently, using our electronic medical record system.
Requirements
  • Master's degree in Nursing or Physician Assistant studies, or equivalent experience and licensure.
  • Current RN and NP/PA licensure in the state of practice, with prescription authority or the ability to obtain it.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant, with a strong background in home health care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong computer skills and familiarity with electronic medical record systems.
