Key Responsibilities- Plan and coordinate care and education for patients with diabetes across a continuum in collaboration with other disciplines, community agencies, and Sentara Healthcare subsidiaries.
- Provide patient, family, and staff education to promote wellness and prevention of complications associated with diabetes.
- Develop and implement diabetes education programs to meet the needs of our patients and community.
- Collaborate with healthcare providers to ensure comprehensive care and education for patients with diabetes.
- Stay up-to-date with the latest research and best practices in diabetes management and education.
- Active VA or Compact RN license.
- BSN degree.
- At least 2 years of experience in diabetes care.
- Certification in Diabetes Care and Education Specialist (CDCES) required within 2 years of employment.
