Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at the Veterans Affairs, Veterans Health Administration. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

Key Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and documenting patient information.
  • Collaborate with healthcare professionals to develop and implement individualized patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and implementing interventions as needed.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance patient care and safety.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and patient satisfaction.
Requirements
  • Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth of the United States or the District of Columbia.
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC).
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with minimal supervision.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse patient populations and healthcare teams.
Preferred Qualifications
  • PACT LPN experience, with a strong understanding of patient-centered care and population health management.
  • Experience working in a healthcare setting, with a focus on providing high-quality patient care and contributing to quality improvement initiatives.
Work Environment

The incumbent will work in a dynamic and fast-paced healthcare environment, with opportunities to contribute to quality improvement initiatives and advance their professional development. The work schedule will be 7:30AM - 4:00PM, with occasional travel required (up to 25%).
