Care Manager Job Summary
We are seeking a dedicated Care Manager to join our team at Sunrise Senior Living. The successful candidate will be responsible for providing exceptional care and services to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods.
Responsibilities
- Build meaningful relationships with seniors and their families by assisting with activities of daily living, attending to individual care needs, and getting to know their unique preferences and personalities.
- Develop and implement Individualized Service Plans (ISPs) and monthly updates in collaboration with the community team.
- Be responsible for your designated group of residents during the shift, knowing where they are and physically checking on them throughout the shift.
- Partner with the community team to ensure compliance with OSHA requirements and promotion of Risk Management programs and policies.
- Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards.
Qualifications
- Dedication to serving seniors with excellent customer service skills.
- A positive attitude, flexibility, and ability to work in a team environment.
- A High School diploma/GED accepted and may be required per state regulations.
- Must maintain certifications in states where applicable.
- Must be at least 18 years old.
- Prior experience working with seniors preferred.
- Ability to make choices, decisions, and act in the resident's best interest.
- Possess written and verbal communication skills.
- Competent in organizational and time management skills.
- Demonstrate good judgment, problem-solving, and decision-making skills.
Estimated Salary: $45,000 - $65,000 per year
Based on geographic location, skills, experience, qualifications, and certifications/licenses, we offer competitive compensation. Our estimated salary range for this role is between $45,000 and $65,000 per year.
