Job Description

At Liberty Hospice, we prioritize delivering exceptional care to our patients and their families. As a Hospice Nurse, you will play a vital role in providing state-of-the-art care and pain management services.

Key Responsibilities:

  • Apply your knowledge of nursing theories and concepts to deliver high-quality patient care.
  • Coordinate physical care with the patient, family, physician, and Hospice team.
  • Communicate effectively with patients, families, and agency personnel.
  • Participate in IDT meetings and quality reviews as scheduled.

Requirements:

  • Current RN licensure in the state of practice.
  • Two years of clinical nursing experience, with at least one year of hospice nursing experience preferred.
  • Knowledge of home health and hospice care programs and the nurse's role in patient care.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and weekend/evening call duties.
