Hospice Nurse

Liberty Hospice is seeking an experienced Registered Nurse to join our team. As a Hospice Nurse, you will provide compassionate care to patients and their families, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care.
Responsibilities:
* Apply nursing theories and concepts to provide individualized care to patients
* Coordinate physical care of patients with family, patient, physician, and Hospice team
* Communicate effectively with patients, families, and healthcare team members
* Participate in IDT meetings and quality review as scheduled by agency
Requirements:
* Current RN licensure in state of practice
* Successful completion of a nursing education from an approved school of nursing
* Two years of clinical nursing experience and at least one year of hospice nursing experience preferred
* Knowledge of home health and hospice care program framework, function, and organization
* Working knowledge of nursing principles and practice according to nursing practice act, agencies policies and procedures, and reimbursement guidelines
* 12 hours of continuing education annually
* CPR certification
