Job Summary:The LPN in Home Health is responsible for delivering patient-centered care, working collaboratively with the healthcare team to develop and implement individualized care plans. You will provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with other members of the healthcare team.
Responsibilities:- Provide quality patient care and perform technical skilled care in the patient's home.
- Instruct patients and their families in the care of the patient and maintenance of a healthy environment.
- Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification is required.
