Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care and perform technical skilled care in the patient's home.

Responsibilities
  • Provide quality patient care and perform technical skilled care in the patient's home.
  • Follow the plan of care and coordinate with other members of the healthcare team.
  • Instruct patients and their families in the care of the patient and maintenance of a healthy environment.
  • Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition.
Requirements
  • Current LPN licensure in the state of practice.
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR Certification is required.
