Job Summary:
As a Home Health Nurse - LPN, you will be responsible for assessing patients' conditions, initiating start of care face-to-face visits, performing evaluation tasks, administering medication as prescribed, and providing patient education. You will also coordinate with the interdisciplinary team regarding care plan progress and make recommendations for devices/tools/medications that might improve patients' quality of life.
Responsibilities:
Assess patients' conditions and initiate start of care face-to-face visits
Perform evaluation tasks, including vital signs and medication review
Administer medication as prescribed by the patient's physician
Dress (or re-dress) wounds and provide patient education
Coordinate with the interdisciplinary team regarding care plan progress
Make recommendations for devices/tools/medications that might improve patients' quality of life
Requirements:
Must be 18 years or older and have successfully completed a FL-approved LPN program from an industry-accredited institution
Must have reliable transportation and proof of auto insurance
Must successfully complete a Level II background check prior to offer and initiation of training
Must complete a competency check and all orientation prior to initiating patient care
Current FL State LPN license is required
Work Environment:
We serve the following counties: Lake, Sumter, Citrus, and Hernando. This is a per diem, contract (1099) position, and you will be working independently with patients in their homes. You will be responsible for managing your own schedule and ensuring timely completion of patient visits.
