Patient Service Coordinator, Home Health Specialist

About the Role:

The Patient Service Coordinator plays a vital role in ensuring seamless care coordination and continuity for our patients. This position is responsible for scheduling visits, communicating with field staff, patients, physicians, and other stakeholders to maintain proper care coordination.

Key Responsibilities:

  • Manage patient schedules, ensuring timely reassignment of visits in case of agent unavailability.
  • Initiate infection control forms as needed and send completed reports to HRD for upload in the worker console.
  • Provide backup support during busy periods, including receiving calls from field staff and assisting with weekly case conferences.
  • Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to relevant staff.
  • Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
  • Process reassigned and rescheduled visits, ensuring supervisory visits are scheduled.
  • Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepare weekly Agent Schedules, performing initial reviews for productivity/geographic issues and forwarding schedules to the Branch Director for approval.

Requirements:

  • Excellent communication skills and ability to interact with diverse individuals.
  • Strong organizational skills and ability to manage/prioritize multiple assignments.
  • At least 1 year of home health experience.
  • Prior packet review/QI experience preferred.
  • Coding certification preferred.
