Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team in King County. As an LPN, you will play a vital role in providing high-quality patient care and supporting our patients in their homes.

Key Responsibilities
  • Assist the Registered Nurse in planning and delivering nursing care to patients in their homes.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Provide education and support to patients and their families to promote self-care and independence.
  • Collaborate with the healthcare team to ensure seamless care transitions and coordination.
Requirements
  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Washington Licensed Practical Nurse licensure.
  • National Provider BLS - American Heart Association certification.
Preferred Qualifications
  • One year of recent hospital, nursing home, or home health experience.
