Job Description

The HCB Wait List Care Coordinator is responsible for establishing person-centered care plans that ensure individuals receive supportive services in a timely and cost-effective manner. This role involves case management, disease management, discharge planning, transition planning, and addressing social determinants of health.

Responsibilities

  • ">
  • Assess individual needs and develop personalized care strategies">
  • Identify and initiate referrals for social services, including financial and community support">
  • Communicate effectively with stakeholders to ensure quality coordinated care">
  • Advocate for individuals and families to ensure their needs are represented and supported">

Required Skills and Qualifications

">

To be successful in this role, you'll need:

  • ">
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, or related field">
  • 1+ years of experience working with individuals with long-term care needs">
  • Intermediate level of computer proficiency, including Microsoft Outlook and Teams">
  • Ability to travel 75% of the time and maintain a valid driver's license">

Location

This field-based position is located in Reno, Ford, Sedgwick counties, and surrounding areas, with local travel up to 75% reimbursed at the current government rate.
