ROLE SUMMARY

The Care Coordinator is tasked with overseeing and managing resident care to guarantee that high-quality services are consistently provided. This role also involves leading the care team effectively.

KEY RESPONSIBILITIES

  • Ensure that all residents receive compassionate and respectful treatment.
  • Safeguard residents from potential accidents and injuries.
  • Take charge of resident safety during emergencies.
  • Act as a primary resource for team members.
  • Facilitate the orientation and training of new care team members.
  • Encourage a positive workplace atmosphere and support skill development among team members.
  • Review and update resident care in accordance with individual service plans.
  • Conduct rounds with medical professionals and relay new directives to staff and families as necessary.
  • Assist in covering staff schedules as needed.
  • Respond promptly to resident call signals and safety alarms.
  • Administer medications to residents following physician orders and established protocols.
  • Process physician orders using electronic medication administration records (eMAR) alongside pharmacy support.
  • Assist in evaluating resident needs.
  • Conduct departmental audits and report findings to the Director of Resident Care.
  • Maintain adequate inventory of medications and supplies for residents.
  • Observe and report any non-emergency changes in resident behavior or condition to the Director of Resident Care.
  • Complete daily documentation and reporting as required.
  • Participate in or lead planned activities organized by Life Enrichment, encouraging resident involvement.
  • Adhere to infection control, safety, emergency, and confidentiality protocols.
  • Participate in on-call rotations as assigned.
  • Foster positive relationships with residents, staff, and visitors.
  • Remain alert and attentive while on duty.

Perform additional duties as assigned within the scope of the role.

QUALIFICATIONS

Education and Experience: A high school diploma or GED is required, along with successful completion of Care Specialist I and II training within 90 days of hire and a minimum of 6 months of relevant experience.

Certificates and Licenses: CPR/First Aid certification is required or must be obtained within thirty (30) days of hire.

Computer Skills: Proficiency in Microsoft Office in a Windows environment is necessary, with the ability to learn company-specific software.
