VNA Health Group is seeking a highly skilled Occupational Therapist to join our team in providing exceptional home health care services.
Key Responsibilities- Assess and evaluate clients for occupational therapy needs, developing and implementing personalized plans of action in collaboration with physicians.
- Document all changes in client condition and update care plans and outcomes accordingly.
- Identify counseling needs and plan appropriate teaching strategies to promote client independence and well-being.
- Monitor quality and appropriateness of services provided, adhering to agency infection control and safety policies.
- Adhere to the organization's policy regarding absenteeism and appearance.
- Occupational Therapy Licensure in NJ with AOTA certification required.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills and effective communication and interpersonal skills.
- Valid NJ driver's license.
Working indoors and outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, and use of phone and tablet for documentation.
