Essential Role in Integrated Healthcare: Medical Respite Care Coordinator/Navigator

In this critical position, you will be part of a dedicated team at LifeBridge Health, serving over 1 million patients annually. Our organization consists of Sinai Hospital of Baltimore, Northwest Hospital, Carroll Hospital, and more.

About the Job

As a Medical Respite Care Coordinator/Navigator, your primary focus will be on supporting medical respite program goals, which include improving pathways to healthcare for at-risk residents of Maryland.

  • You will function as a system navigator and care coordinator for medical respite patients, facilitating access to primary and specialty healthcare, dental/oral health, behavioral health, diagnostic services, and social services.
  • Participate in triage and intake processes for medical respite patients, ensuring timely and effective support.
  • Provide intensive case management and advocacy for at-risk patients, including resource navigation, application assistance, transportation, social supports, and housing stability.
  • Promote person-centered care by partnering with other providers and services within the community.
Key Responsibilities
  1. Maintain working knowledge of State of Maryland and Federal assistance programs, including public insurances, financial assistance gap programs, Social Security Administration, food stamps, energy assistance, and others.
  2. Aid patients with private and public assistance program applications, streamlining the process.
  3. Participate in community workgroups and continuing education opportunities to enhance patient advocacy, healthcare advancements, and entitlement programs.
  4. Conduct needs assessments to identify barriers to obtaining healthcare services, available resources, and potential diversion of emergency department services.
  5. Support clinical and procedural training for other healthcare team members related to care coordination and navigation processes.
  6. Participate in quality assurance/performance improvement measures and activities, contributing to the development and reporting of essential metrics.
  7. Coordinate referrals, specialty visits, and diagnostic services, ensuring proper documentation and patient education.
  8. Track care coordination activities via patient registries and specialized databases.
  9. Prepare monthly reports detailing care coordination statistics, referrals, individuals served, and relevant outcomes.
  10. Offer mentorship and training to integrated staff, partnering entities, and professional students.
Requirements
  • Licensed nurse in the State of Maryland (RN or LPN) with Human Services field experience preferred.
  • Bachelor's Degree from an accredited college or university (preferred but not required) or equivalent related experience.
  • Current BLS Certification from the American Heart Association.
  • Strong organizational skills and attention to detail.
  • Excellent interpersonal communication and relationship-building skills.
  • Superior written and verbal communication skills.
  • Ability to work effectively with various cultural and diverse individuals.
  • Bilingual English/Spanish preferred.
  • Understanding of policy and regulations concerning Maryland public assistance programs, specifically Medicaid, Primary Adult Care Program, Medicare, and the Social Security Administration.
