We are seeking a highly skilled and compassionate Nurse Navigator to join our Mobile Health Clinic team at Sparrow Health System. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and navigation services to underserved communities in our service region.
Key Responsibilities- Oversee the logistics of the Mobile Health Clinic, working closely with transportation services, resource coordinators, and healthcare providers to coordinate outings and services.
- Coordinate the delivery of screening services, immunizations, behavioral health consultations, prenatal care, and primary care physician visits to medically underserved communities.
- Clinically screen, assess, and evaluate patients for attending primary care physicians.
- Responsible for charting and tracking patient information in our electronic medical record system, documenting follow-up appointments and a plan of care in a clear and concise manner.
- Provide education, information, and support to patients based on their current conditions, concerns, or issues.
- Collaborate with the multidisciplinary team to identify issues that may impact medical outcomes.
- Assist patients and families with access to appropriate resources and services based on a thorough knowledge of community resources and eligibility requirements.
- Communicate effectively with patients, families, healthcare team members, and others in a professional, diplomatic, and empathetic manner.
- Provide clear, concise, and timely written documentation on patient medical records and departmental records.
- Current Michigan license to practice as a Registered Nurse.
- Community Health Worker certification preferred.
- Minimum 5 years of bedside nursing experience.
- Graduate of an accredited nursing program.
- BSN preferred.
- Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
- Ability to function in a team environment, including excellent written and verbal communication skills.
- Ability to work in an electronic medical record environment.
- Ability to travel locally.
- Self-directed and independent work practices.
