We are seeking an experienced Care Coordinator to join our team. This role involves supervising care at clients' homes, scheduling, consultations with family and clients, communication with clients and caregivers, and working cooperatively with other Case Managers and team members.

The ideal candidate will have experience in social work and/or case management supervision, or a similar background as a business owner/manager. Excellent verbal and written communication skills, excellent customer service skills, and great phone presence are essential.

The salary for this position is approximately $50,000 per year, plus benefits including health insurance, PTO, bonus, company car, paid gas, and routine maintenance and oil changes.
