Job Summary:

We are seeking a highly skilled and compassionate Pharmacist to join our team at Synchrony Health Services. As a Pharmacist, you will play a vital role in providing exceptional patient care and medication management services in a long-term care setting.

Responsibilities:

• Prepare and dispense medications according to policy and legal requirements
• Monitor patient drug therapies and provide pharmacological information to healthcare teams
• Assist in the maintenance of records, including pharmacy files, patient profiles, and inventories
• Control medications by monitoring drug therapies and advising interventions
• Complete pharmacy operational requirements, including organizing and directing technicians' work flow
• Manage workflow processes, including pharmacist verification, labeling/packaging, and quality assurance
• Utilize computer systems to screen for drug interactions, therapeutic duplication, and other potential issues
• Provide pharmacological information and counseling to patients and healthcare professionals
• Develop staff pharmacological knowledge through clinical programs and training

Requirements:

• Bachelor's of Science (B.S.) or Doctor of Pharmacy (Pharm.D.) degree accredited by the Accreditation Council for Pharmacy Education (ACPE)
• North American Pharmacist Licensure Exam (NAPLEX) and Multistate Pharmacy Jurisprudence Exam (MPJE) certification
• State-specific license where required
• Five (5) years' experience in a pharmacy environment, preferably in healthcare or senior living industry
• Exemplary computer skills, including knowledge of Microsoft Office Suite products
