We are seeking a passionate and dedicated Anticoagulation Pharmacist to enhance our thriving Ambulatory Care Pharmacy team at the Meridian Medical Center.
In this vital role, you will be at the forefront of patient care, working in an established clinic with a high patient volume and a commitment to exceptional service.
You'll be part of a forward-thinking team that values innovation, collaboration, and a patient-centered approach.
Key Responsibilities
- As an Anticoagulation Pharmacist, you'll play a pivotal role in our Heart and Vascular clinic, primarily dedicated to the Meridian Medical Center. You will also support patients managing their care remotely, ensuring they receive top-notch service regardless of their location.
- You will work independently while collaborating with a multidisciplinary healthcare team to optimize patient outcomes. Your expertise will be crucial in medication management, allowing you to build lasting partnerships with patients and healthcare providers alike.
- In our diverse Ambulatory Pharmacy Department, you'll join pharmacists specializing in various fields, including internal medicine, family medicine, endocrinology, heart failure, lipidology, and more. Together, we strive to improve healthcare quality and patient outcomes through integrated, accessible services.
Requirements
- Degree from an accredited pharmacy program
- Valid Idaho Pharmacist License
- Current Basic Life Support (BLS) Provider Certified through American Safety and Health Institute, American Heart Association, or American Red Cross.
Preferred Qualifications
- Previous anticoagulation or ambulatory experience
- Experience managing anticoagulation therapies in a clinical setting
- Clinical management of anticoagulation therapies
- Completion of a PGY1 pharmacy residency program with ambulatory-focus or equivalent ambulatory care experience
- Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), Certified Anticoagulation Care Provider (CACP)
