Job Summary

St. Luke's Health System is seeking a dedicated and detail-oriented Outpatient Pharmacist to join our Home Delivery Department. This role is essential in ensuring the safe and effective delivery of medications to our patients in the comfort of their homes.

Key Responsibilities

Review and verify medication orders for accuracy, completeness, and appropriateness prior to dispensing.
Provide medication counseling and education to patients and caregivers regarding proper medication use, potential side effects, and interactions.
Collaborate with healthcare providers to optimize patient medication regimens and address any concerns.
Oversee the preparation and packaging of medications for home delivery, ensuring compliance with safety standards and regulations.
Maintain accurate patient records and documentation in compliance with pharmacy laws and organizational policies.

Requirements

Degree from an accredited pharmacy program
Valid Idaho Pharmacist License
