Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at ChristianaCare. As a key member of our team, you will provide high-quality, patient-centered care to geriatric patients in the community.

Responsibilities
  • Deliver skilled nursing care and prescribed treatment to patients in their homes.
  • Develop and implement individualized care plans that are goal-oriented and patient-centered.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.
  • Provide education and support to patients and families to promote health and well-being.
Requirements
  • Current DE RN license or compact state license.
  • 1 year of RN experience, with previous Home Healthcare and Med-Surgical experience preferred.
  • IV proficient or willing to train, and perform IV functions.
  • BSN required; ADN candidates must obtain BSN within 3 years of start date.
