About the Role

We are seeking a highly skilled Patient Care Manager to join our team at Heartland Home Health.

This role involves overseeing the day-to-day operations of our home health sites, ensuring the provision of quality care to patients and facilitating relationships between referral sources, patients, caregivers, and employees.

Key Responsibilities
  • Develop and implement individualized plans of care for clients, ensuring they are executed as written and reassessed as needed.
  • Manage patient transfers to inpatient facilities, ensuring accurate documentation is completed.
  • Lead site operations in the absence of senior management, overseeing clinical and administrative functions.
  • Participate in on-call nursing schedules and provide direct bedside nursing care when required.
Requirements
  • A minimum of three years' experience in direct patient care, preferably in a clinical setting.
  • An Associate's degree in Nursing from an accredited school, or equivalent qualification.
  • Current RN licensure in the state of practice and CPR certification.
  • Strong leadership and communication skills, with the ability to effectively operate nursing assessment tools and office equipment.

Compensation

We offer a competitive salary range of $85,000 - $110,000 per annum, depending on experience and qualifications.
