**Job Summary:**
We are seeking a skilled Clinical Specialist to join our team at First Connect. As a Clinical Specialist, you will be responsible for providing high-quality patient care and support in a dynamic and fast-paced environment.
**Responsibilities:**
* Provide direct patient care and support to patients with complex medical needs
* Collaborate with healthcare teams to develop and implement individualized care plans
* Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
* Maintain accurate and detailed records of patient interactions and care plans
**Requirements:**
* Minimum 2 years of experience in a clinical setting
* Current AHA, BLS, and ACLS certifications
* Proficiency in CERNER charting system
