We are seeking a highly motivated and compassionate Community Support Worker to join our team at the University of New Mexico - Hospitals. Key Responsibilities- Coordinate and provide services and resources to promote rehabilitation, recovery, and resiliency.
- Identify and address strengths and barriers to independent functioning in the community.
- Support individuals and families in crisis situations and provide personalized interventions.
- Develop and facilitate crisis management plans and ensure adherence to Hospitals and departmental policies and procedures.
- Collaborate with medical providers, multidisciplinary team members, and community resource providers to assist patients in meeting treatment goals.
- Facilitate the development of skills in activities of daily living, interpersonal coping, socialization, and community functioning.
- Document all client encounters and maintain current and comprehensive client files.
- Bachelor's Degree in a related field.
- 1 year of directly related experience.
- CPR for Healthcare/BLS Provider certification within 30 days.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
This position requires working in a dynamic and fast-paced environment with minor hazards, including physical risks, dirt, dust, fumes, and noise.
