Job Description

At Principle Choice Home Healthcare, we are seeking a highly skilled and compassionate Licensed Clinical Social Worker to join our team. As a key member of our interdisciplinary team, you will provide social work services to patients in their place of residence, working closely with physicians and other healthcare professionals to ensure comprehensive care.

Key Responsibilities:

  • Provide social work services to patients, including assessment, planning, and implementation of care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination.
  • Develop and maintain relationships with patients, families, and caregivers to promote patient-centered care.
  • Participate in patient care conferences and contribute to the development of care plans.
  • Document all patient services provided, maintaining accurate and timely records.

Requirements:

  • Master's or doctoral degree in social work from a Council on Social Work Education-accredited program.
  • Current, unrestricted state license to practice as a social worker.
  • Basic Cardiac Life Support (BCLS) certification.
  • 1-2 years of experience in home health or a related field.
  • Active Oklahoma driver's license and current vehicle insurance.

Working Conditions:

The Licensed Clinical Social Worker will work in a variety of settings, including patient homes, clinics, and healthcare facilities. The position requires moderate physical exertion, including extensive driving and assisting patients with transfer activities. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
