We are seeking a skilled Licensed Practical Nurse to join our Dialysis Center team in Park Ridge. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the success of our operations.
Responsibilities- Patient Care: Set up and deliver dialysis treatment, monitor patient status for changes in treatment parameters, and document on dialysis flow sheet and patient chart.
- Team Collaboration: Provide ongoing support to RNs for care of primary patients, including completion of monthly multidisciplinary forms, monitoring and updating immunization and vascular access records, performing quarterly medication reviews, biannual emergency disconnect teaching, and updating patient Kardex and records as needed.
- Project Management: Demonstrate organizational ability and prioritize actions based on clinical staff and unit needs.
- One year of clinical experience, acute care preferred.
- Strong customer service, computer, and communication skills.
