Key Responsibilities- Evaluate and coordinate care for residents, ensuring that their needs are met and their goals are achieved.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct regular assessments and evaluations to identify changes in resident needs and make recommendations for adjustments to care plans.
- Communicate effectively with residents, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance the overall quality of care.
- Graduate of an accredited nursing program.
- Current RN licensure in the state of practice.
- Minimum of one year of experience in a healthcare setting, preferably in a long-term care or geriatric setting.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
