About the Role:
We are seeking a skilled and compassionate Private Duty Nurse RN to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will play a vital role in providing high-quality, patient-centered care to our clients in the comfort of their own homes.
Key Responsibilities:
- Implement and manage individualized care plans in collaboration with our interdisciplinary team
- Conduct thorough patient assessments and coordinate care with other healthcare professionals
- Administer medications, treatments, and therapies as prescribed by physicians
- Document patient care and progress in our electronic health record system
- Provide education and support to patients and their families on health promotion, disease prevention, and management
- Collaborate with our Case Management team to ensure seamless transitions of care
Requirements:
- Active, unencumbered RN license in the state of practice
- Current CPR certification from American Red Cross or American Heart Association
- TB skin test (current within last 12 months)
- At least six months of prior hands-on nursing experience (preferably in home health or hospice care)
- Reliable transportation and a valid driver's license
