Job Title: Private Duty Nurse LPN

Job Summary:

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans in collaboration with the patient's healthcare team.
  • Conduct regular patient assessments and update care plans as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current, unrestricted LPN license in the state of practice.
  • Minimum 6 months of recent experience as an LPN in a clinical care setting.
  • Home health experience preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
