Key Responsibilities:

Clinical Operations: Develops and implements clinical operations for a specific location, ensuring high-quality patient care and efficient service delivery.

Patient Care: Conducts assessments and reassessments, updates care plans, and interprets patient needs to ensure effective care coordination.

Staff Management: Manages caregiver assignments, oversees delivery of care, and instructs and guides clinicians to promote effective performance and quality home care services.

Communication: Coordinates communication between team members, attending physicians, and caregivers to ensure seamless care delivery.

Leadership: Supervises all clinical employees assigned to a specific location, handles necessary employee corrective action and discipline, and conducts patient care assignments and staff management activities.

Requirements:

  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice, or equivalent environment.

Benefits:

The compensation range is $76,800 - $105,800 per year with a bonus incentive plan. Benefits include medical, dental, vision, 401(k), time off, short-term and long-term disability, life insurance, and more.
