Job Summary

Aveanna is seeking a skilled Licensed Practical Nurse (LPN/LVN) to join our team in providing high-quality patient care in the comfort of their own homes. As a LPN/LVN, you will be responsible for delivering and coordinating quality patient care in compliance with physician orders, while maintaining confidentiality and adhering to professional boundaries.

Key Responsibilities
  • Deliver and coordinate quality patient care in compliance with physician orders
  • Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate
  • Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner
  • Participate in and update the nursing care plan
  • Take appropriate nursing action based on assessment and achieve expected outcomes
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements
  • Must be able to speak, write, read, and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
