The Licensed Practical Nurse will be responsible for forming relationships with residents, their families, and physicians to devise care plans that reflect critical thinking and a strategic perspective. Responsibilities:
  • Develop and implement individualized care plans for residents
  • Monitor and administer medications as prescribed by physicians

  • Collaborate with the Manager on Duty to ensure seamless, quality care is delivered around the clock
Requirements:
  • Proof of valid LPN license in good standing from the State Of Pennsylvania
  • CPR Certification (options available to help obtain with facility covering costs)
  • Ability to perform physical tasks required in this position

  • Minimum 1 year experience in assisted living or other healthcare setting
