We are seeking a highly skilled Clinical Pharmacist to join our Acute Care team at Novant Health. As a key member of our multidisciplinary team, you will be responsible for delivering optimal medication management across all dimensions of care, every time.
Key Responsibilities- Facilitate pharmaceutical care services, medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
- Complete critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems.
- Partner as an active member of the multidisciplinary team and collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care.
- Make appropriate evidence-based, patient-centered medication recommendations.
- Participate in the management of medical emergencies.
- Provide discharge medication review, reconciliation, and counseling as appropriate.
- Maintain proficiency in and actively engage in central dispensing functions and operational programs.
- Oversee pharmacy operations and technician practice/activities.
- Maintain proficiency in preceptor roles and actively emulate education/mentorship skills.

We are seeking a Clinical Pharmacist who shares our commitment to quality care and is passionate about delivering exceptional patient outcomes.
Qualifications- 4-Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.
- Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required.
- Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required.
- Board of Pharmacy Specialties Certification or other approved certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.
