Nurse Preceptor

Job Summary:

VNS Health is seeking a skilled Nurse Preceptor to join our team. As a Nurse Preceptor, you will play a critical role in the onboarding process of newly hired Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) in our Home Health Care division.

Key Responsibilities:

• Provide individualized instruction and guidance to new hires during the onboarding period.
• Conduct joint visits with field clinicians to observe and instruct in current/new techniques/procedures/treatments and overall assessments in alignment with VNS Health policies and procedures.
• Support the knowledge and skills development of new RNs and LPNs, individually and in small groups, including but not limited to documentation, case management, clinical skills, patient/family-centered care, reimbursement, time management, customer experience, etc.
• Participate in regularly scheduled preceptor team meetings that are focused, productive, promote 'consensus' decision-making and enables the team to achieve desired performance results.
• Provide ongoing feedback to branch management regarding individual nurse's learning progress and performance.
• Assume responsibility for personal professional growth through attendance at in-services, internal and external training/workshops/conferences, select journal readership, and continued formal education.
• Travel to patients' homes and/or other facilities with varying environments to deliver direct care to patients.
• Transport and utilize required medical equipment and supplies using VNS Health designated/supplied carrying case.

Requirements:

• A license and current registration to practice as a Registered Professional Nurse in New York State required.
• Valid driver's license required.
• OASIS certification preferred.
• Certification in Hospice and Palliative Care Nursing preferred.
• Bachelor's Degree in Nursing OR Associate's degree in Nursing required.
• Four years' clinical home care experience required.
• Minimum of two years full-time clinical home care experience required.
• Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred.
