Hospice RN Case Manager

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in their final stages of life.
Responsibilities:- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and performing various medical procedures as prescribed by physicians.
- Collaborate with the interdisciplinary team to coordinate patient care and ensure seamless transitions between care settings.
- Communicate effectively with patients, families, and healthcare providers to ensure that patient needs are met and that care is delivered in a timely and efficient manner.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote high-quality patient care.
- Current RN licensure in California.
- Minimum of one year of experience as a professional nurse, preferably in hospice or palliative care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
- Basic computer skills and proficiency in electronic medical records.
