Licensed Practical Nurse

The Licensed Practical Nurse (LPN) is a vital member of our interdisciplinary team, providing direct patient care under the supervision of a registered nurse. This role requires a comprehensive understanding of nursing practices, with a focus on delivering high-quality care to patients with complex needs.
Key Responsibilities- Provide direct patient care, including specialized duties related to end-of-life care, as defined in the State Nurse Practice Act.
- Implement current nursing practices, following a comprehensive assessment and the plan of care, to ensure optimal patient outcomes.
- Document patient services accurately and timely, reflecting the plan of care and any changes or updates.
- Assess and provide patient and family/caregiver education, addressing diagnosis and self-care plans.
- Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary group.
- Use and prepare equipment and supplies effectively, adhering to aseptic technique.
- Provide appropriate pain/symptom management, evaluating patient response to treatments/medications.
- Participate in personal and professional growth and development, contributing to the organization's quality assessment and performance improvement teams.
- Graduate of an accredited practical nurse or vocational nursing program.
- One year of nursing experience, preferably in community health/hospice or medical/surgical settings.
- Current licensure as an LPN/LVN in the State.
- Compliance with accepted professional standards and practice.
- Good verbal and written communication, and organization skills.
- Understanding of the hospice concept and the needs of the terminally ill.
- Compliance with company policies, procedures, and processes, as well as Hospice and Homecare rules and regulations.
- Dependability and flexibility in work schedule.
- Compliance with harassment and discrimination policies.
- Fostering good working relationships.
- Licensed driver with a licensed automobile, insured in accordance with organization/state requirements, and in good working order.
- Willingness to submit to a criminal background check and periodic drug/alcohol screening.
- Ability to see color changes on a patient's skin, listen to heart and lung sounds through a stethoscope, and feel skin temperature and palpate pulses.
- Vital sensory abilities, including near vision, far vision, perceptual speed, visual color discrimination, depth perception, and hearing sensitivity.
- Finger dexterity, arm-hand steadiness, and manual dexterity, with the ability to manipulate equipment safely and effectively.
- Ability to communicate with patients, families, and other healthcare workers, with speech clarity, speech recognition, and written expression.
- Ability to lift heavy items frequently, with the capacity to lift and carry up to 50 lbs. with or without assistance.
- Ability to spend prolonged periods standing, walking, bending, and stretching.
- Ability to deal with stressful situations and grieving families, while maintaining compassion without becoming personally involved.
