Job Title: Hospice RN Case ManagerJob Summary:

We are seeking a compassionate and skilled RN Case Manager to join our team at Compassionate Care Hospice. As a RN Case Manager, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of our patients and their families.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine hospice needs
  • Develop and implement individualized care plans to meet patient needs
  • Provide education and support to patients and families on end-of-life care and symptom management
  • Collaborate with interdisciplinary team members to ensure seamless care coordination
  • Document patient information and progress in a timely and accurate manner
Requirements:
  • Current RN license in California
  • Minimum of 1 year of experience in hospice or palliative care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
