Case Manager

WellLife Network is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for the daily operations of our Supported Housing Program.
Key Responsibilities:- Coordinate and provide case management services to ensure the quality of care for our consumer population.
- Drive self and/or agency vehicle to visit apartments, assess, observe, and evaluate physical plans and residents' functioning abilities and needs.
- Act as a liaison with community health, mental health, educational, vocational, and recreational programs.
- Supervise and collect urine samples, ensuring accurate testing procedures.
- Provide linkage services with detoxification and rehabilitation treatment centers.
- Assist consumers in the housing selection process, including participation in the selection of adequate furnishings and working with real estate agents.
- Facilitate sublease and rental payment agreements, as well as housing subsidies, food stamps, and other benefits.
- Assist consumers in linking to community support services in identified areas of need.
- Familiarize consumers with their rights as tenants and assist in tenant-landlord communication.
- Develop individual support plans to address areas such as self-care, money management, medication management, daily living skills, and symptom management.
- Maintain all required written documentation, including consumer records and program records.
- Identify maintenance and safety needs of assigned apartments.
- Report and document all incidents to immediate supervisors and/or the Director of MHS within 24 hours.
- Be on-call 24 hours a day, seven days a week, to respond to emergencies.
- Provide weekly supervision of assigned staff, delegating tasks and providing professional direction and technical assistance.
- Participate in meetings and trainings at various sites, following different routes of travel to worksites.
- Lift/carry packages and boxes of supplies, as well as clients if necessary.
- Count, add, subtract, and manage money.
- Communicate information with all staff and supervisors verbally and in writing for the purpose of giving instructions, providing feedback, and adjusting work practices.
- Participate in activities of other staff members in their absence or during periods of staff shortage.
- Represent the agency at meetings and trainings not otherwise specified.
- Work flexible schedules, as work schedules are subject to change.
Qualifications:
Bachelor's degree in Social Services preferred. Experience working with mental health consumers with/without co-occurring addictive disorders is required. A New York State driver's license and access to a vehicle are also required.
