Home Health Clinical Manager

AccentCare, Inc. is seeking a skilled and compassionate Home Health Clinical Manager to join our team. As a key member of our leadership team, you will be responsible for overseeing the clinical operations of our home health services, ensuring the highest quality of care for our patients.
Key Responsibilities:
- Provide clinical leadership and oversight to our home health team, ensuring compliance with regulatory requirements and agency policies.
- Coordinate patient care with an interdisciplinary team approach, ensuring the development and implementation of individualized plans of care.
- Supervise and mentor our clinical staff, providing coaching and guidance to ensure the delivery of high-quality patient care.
- Collaborate with clinicians and physicians to ensure seamless transitions of care and effective communication.
- Monitor and analyze clinical data to identify trends and areas for improvement, implementing corrective action plans as needed.
Requirements:
- Current licensure as a Registered Nurse in the practicing state.
- Possess and maintain CPR certification.
- Previous experience in home care setting with two years management or supervisory experience, preferred.
- Proven ability to work within an interdisciplinary setting.
- Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements.
