Job Title: LPN II Nursing Home

Covenant Health (Tennessee) is seeking a skilled LPN II to join our team at Claiborne Medical Center. As a Licensed Practical Nurse II, you will play a vital role in providing high-quality patient care in our nursing home setting.

Job Summary:

The LPN II Nursing Home will be responsible for administering routine services in caring for subacute, convalescent, and chronic residents. You will participate in planning, implementation, and evaluation of nursing care, directing and supervising CNAs assigned to your wing. Your duties will include maintaining continuity of resident care, prioritizing nursing tasks, and assigning residents to CNAs. You will also supervise and assist nursing staff in planning care for each resident, assist with orientation of new personnel, and make frequent rounds on residents to observe and record symptoms and/or reactions to treatments and changes in condition.

Responsibilities:
  • Maintain continuity of resident care during respective tours of duty and prioritize nursing tasks.
  • Assign residents to CNA and supervise and assist nursing staff in planning care for each resident.
  • Assist with orientation of new personnel and make frequent rounds on residents to observe and record symptoms and/or reactions to treatments and changes in condition.
  • Supervise private sitters assigned to residents on units and maintain and perform sterile and clean dressings, and monitor wounds.
  • Chart appropriately and complete a monthly summary on assigned residents using information from the comparison of present resident care with documented plan of care.
  • Order treatment online and transcribe physician orders.
  • Assure that 2 licensed persons sign off all physician's orders and that a 990 form is completed for all verbal or telephone orders.
  • Perform other duties as assigned.
Requirements:
  • Current TN LPN license required.
  • CPR certification required within 1st year.
  • One (1) year experience as an LPN.
